Please fill out this form, print it with your browsers print function, and then either bring it to Studio Be along with your payment or mail it with your payment to: Studio Be Yoga, 2077 30th St., Boulder CO 80301.

If you are registering for single sessions you must pay the entire cost upon registration. If you are taking the entire course you may either pay the full cost upon registration or make a $50 non-refundable deposit upon registration and pay the balance at the time of the workshop. Make checks payable to Studio Be Yoga.

There are three different registration options; the entire workshop, just the Saturday class, or just the Sunday Class. Mark your choice below with an "X" in the box beside your choice. Choose only one option.
